We have a wonderful opportunity available to join our team at Zoe’s Place Baby Hospice in Middlesbrough. Zoe’s Place is a baby hospice specialising in respite care for children 0-5 years of age with a life limiting or life threatening condition. We provide complete holistic care for children and their families. As a nurse in Zoe’s Place you have the time to care. We provide one-to-one care for all children in a relaxed, fun, home from home environment. We are working towards providing end of life care and provide a bereavement service. Main duties of the job

Provide individualised care.
Assess, plan, implement and evaluate care, working in partnership with the family at all times.
Safe transcribing, administration, storage and control of all medication.
All records are accurate, timely and confidential in accordance with the NMC Professional Code and the hospice policy and procedures.
Safe storage or all data and records in accordance with policy and procedures.
Adhere to Zoe’s Trust policies including ICP.
Adopt the role of link worker/ primary nurse
Attend and participate in an annual appraisal.
Confidently and competently take charge on the shift.
To be able to communicate effectively with all members of the team and wider MDT.
Attend clinical supervision.
Complete risk assessments.
Understanding of the safeguarding polices.
Participate in the induction of new staff to Zoe’s Place.
Mentor student nurses during their placement at Zoe’s Place.
Play an active part in the Clinical Governance framework within the hospice.
Support the family at the time of death, respecting an individuals beliefs, community, culture and religion.
To participate with the Nurse on call rota within the hospice.
Constantly improve and develop practice.
To be an advocate for the child and family.

MAIN RESPONSIBILITIES

To actively encourage a home from home family atmosphere within Zoe’s Place Baby Hospice.
Participate in the assessment, care planning, implementation and evaluation of nursing care to children with individual health needs, working in partnership with the family at all times.
To ensure the safe transcribing, administration, storage and control of all medications in accordance with the policy and procedures.
To liaise and communicate effectively with all necessary personnel to ensure accurate and safe information sharing in accordance with the Data Protection Act (2018), the Caldicott Principles, the GDPR regulations and the hospice local policy and procedures.
To ensure that all the child’s and family data is recorded accurately, timely and confidentially in accordance with the NMC Professional Code and the hospice policy and procedures.
To ensure the safe storage or all data and records required for the role in accordance with policy and procedures.
To be involved in the assessment of staff competence and in the training and mentoring of staff as appropriate to the role.
To maintain own professional development with support from senior staff and use evidence to underpin practice.
To adhere to Zoe’s Trust policies, procedures, guidelines and standards at all times and to promote these to others.
To maintain NMC professional registration and revalidation requirements in accordance with NMC guidelines.
Adopt the role of link worker/ primary nurse as required by Zoe’s Place Hospice.
Take advantage of training opportunities to further develop your skills to enhance your role within Zoe’s Place including all mandatory training sessions.
Attend and participate in an annual appraisal and regular reviews of performance and objectives, identifying gaps in knowledge/skills and developing a plan to enhance service and self development.
To encourage and promote the philosophy of Zoe’s Place and present a good public image, to co-operate with all staff in maintaining good relationships with outside agencies in order to uphold the charitys image and to win increased support for its work.
To be able to confidently and competently take charge on the shift and manage the team throughout the shift.
To provide effective and accurate hand over to colleagues ensuring a safe transfer of information to enable the ongoing care needs to be met.
To be able to communicate effectively with all members of the team and to develop excellent relationships with the multi-disciplinary teams (MDT) both internal and external to the hospice.
To participate in mandatory Clinical supervision as directed by the Head of Care and to take responsibility for the supervision of the support workers within the team.
To attend staff meetings and participate in other MDT meetings as appropriate.
To undertake risk assessments and management to ensure that staff, families, children and visitors are safe.
To be aware of the safeguarding children and safeguarding adults procedures and policy within Zoe’s Place baby hospice, to attend all safeguarding training and to adhere to the Local Safeguarding Partnership Procedures at all times.
To participate in the induction of new staff to Zoe’s Place.
To act as a role model showing example in regard to behaviour, attitude, conduct and appearance.
To mentor, supervise and support student nurses during their placement at Zoe’s Place.
To be able to carry out effective complaint, accident and incident reporting.
To play an active part in the Clinical Governance framework within the hospice.
To be aware of the Infection Prevention and Control policies and work in accordance with IPC procedures at all times.
To maintain correct standards of uniform and personal appearance as outlined in the policy.
To promote the speciality of palliative care and evidence based practices and develop the service within the sphere of responsibility.
To be able to support the user and the family at the time of death, respecting an individuals beliefs, community, culture and religion.
To be aware of, and responsive to, the changing nature of the Trust and adopt a flexible and pro-active approach to work.
To participate with the Nurse on call rota within the hospice.
Ensure your role has a positive influence on key performance indicators.

Experience

Essential

Ability to practice within the scope of the NMC code of professional practice
Experience working with children with complex health needs

Awareness of safeguarding children procedures
Understanding of holistic and person centred care.
Experience in relevant clinical area
